A worker at Certifit Incorporated in San Antonio was preparing a loading dock for a delivery when the delivery truck backed over him.
Investigators with the Occupational Safety and Health Administration said the auto body parts supplier is being cited for serious violations that compromised worker safety, including failure to make sure someone was available to give first aid.
The Occupational Safety and Health Administration cited Salt Lake City, Utah-based CertiFit Inc. for nine serious safety violations at the company’s facility in San Antonio. OSHA initiated the inspection after an employee, 40-year-old Solomon Roberts, was killed when a delivery truck backed into a loading dock.
OSHA found the company failed to ensure that a trained person in first aid is present, provide training on evacuation procedures, repair and maintain electrical equipment and rolling ladders.
