This ASA health insurance plan will provide affordable health insurance coverage for your employees and their families. It will include multiple plans with a varying degree of coverage and large employer group plan mobile applications and wellness tools for your members' utilization. Those interested will also have a single composite rate for everyone versus the age banded rates.
Four plan options with varying coverage and price are available:
- CIADA Anthem Blue Preferred - PPO with a $1,000 individual deductible
- CIADA Anthem Lumenos HSA 20 - PPO with a $2,500 deducbtle with HSA
- CIADA Anthem Blue Classic - PPO with a $4,000 individual deductible
- CIADA Anthem Lumenos HSA 24 - PPO with a $5,000 deductible and HSA
The pricing will start as low as $297.00 per employee per month for the Lumenos HSA 24. The employer will be responsible at least 50% of the lowest plan premium.
Dental and Vision will also be offered.

Some of the requirements are:
- You must have at least 5 employees to enroll in the ASA Plan.
- You must be an ASA member
- The Employer will be required to pay at least 50% of the lowest premium option for the health insurance.
- You will be required to contract with Management Solutions and use them for your payroll services. (This is the approved method we use to get large group plan options for the Association)
- Any worker's compensation policy you have will need to be transferred to Taggart and Management Solutions before the end of your policy term.
